It does not look like an injury, but rater Hole-in-the-Head Syndrome which affects primarily gouramis, angelfish, discus, oscars and large South American cichlids.

One of the causes is poor water quality, I would suggest that you check your water parameters more often, making sure the ammonia and nitrite levels are zero. Does your tank have adequate filtration, and how often do you do partial water changes, in order to keep these levels in check.

Heal-all is a "medicine", but it's very natural so it doesn't do any harm to your good bacterias. It promotes healing of wounds and infections. Good product, as far as I can tell.

Can't remember who makes it, but will find out if you're interested.

Heal-all produced by Bio-Elite - check it out on their website www.bio-elite.com
